I don't have a straightforward answer for your problem, but I have two
suggestions: (1) use sysinternal's "Process Explorer" to inspect
what's going on with the DLLs and (2) try the same test case with
another Lisp like Allegro CL or Lispworks. (Possibly using their
native APIs, in case it's a CFFI bug.)
